LICENSE APPLICATION FORMS

The following will help to clarify the procedures and content needed on the license application. Please make sure the following steps are taken prior to the racer forwarding it to the Division Office.

1. Make sure all the general information is complete (i.e. name, address, phone numbers, state drivers license number with expiration date, NHRA membership number with expiration date, eliminator, type of car, previous experience, date of last competition and location, signature of applicant, and date).

2. Circle each eliminator in appropriate box for which the application applies.

3. Put an "X" in the appropriate box designating the category in classes 3 and 4 for which the application applies.

4. Prior to any test runs applicant must present an original (not a copy) NHRA physical form to the track official/manager. Please use forms dated 2004 to current. All other forms are outdated.

5. Complete all information in the test run section (i.e. date [make sure the dates on the time slips match the dates on the test runs listed on the application], location, make/model/class of vehicle used for the test runs, cockpit orientation test and signature of the individual that performed the test, e.t. and mph, two licensed driver signatures as witnesses accompanied by their license number, category and expiration date and track manager signature accompanied with any comments).

6. Issue the time slips for all test runs to the driver for the purpose of submitting them to the Division office with the application. All of the above needs to be completed properly to receive a competition license. If the above items are not completed properly, the application will be returned to the competitor as an incomplete application form.
